How to Write a Prescription for a Compounded Medication
The prescription should begin with the phrase “Compounded Medication” followed by the specifics of the prescription itself. The typical format is as follows:
Prescriptions can be called in to (305)456-7500 or faxed to (305) 363-5482 with the exception of CII prescriptions which must be hand written on a controlled substance prescription form.
